Please be sure to bring your current insurance cards and a photo ID with you to each visit. We will need to keep current copies in your records.
With the constant changes to insurance plans, it is impossible for us to know your specific insurance coverage. We will do our best to help you interpret your healthcare benefits and coverage requirements, however, it is your responsibility to understand which services are covered and which are not covered under your plan. Also, it is your responsibility to identify any coverage changes that may be initiated by your employer or health care plan.
We encourage you to contact your insurance company prior to your appointment if you have specific questions.

If you are a self-pay patient; whether we do not par with your insurance, you do not have insurance, or do not wish to use your insurance for our services, please read the “Good Faith Estimate” information as outlined in the No Surprise Act.
We would not want anyone to be denied medical care because of financial hardship. Please contact our billing department to discuss options for payment.
